Where did my form go?!?!

    Hi There, Please help ASAP. 

    Our patient registration form (medical history) has disappeared.

    I found only two forms related to your account sanclementesmiles, they are Adult New Patient Form  Search  and Child New Patient Form, but I could not find in your account one with the name "new patient registration/medical history" or similar. 

    Do you have the link to your form or the form ID so we can search it on our end? 

    It may be that you have created your form using a guest account and it has been already deleted, do note that guest accounts are very limited.
