Can I create a form that has a comments section, form fields and payment button all in one form?

I need to create a form that looks like this:

Choose Design (drop down list)

Choose Stain (drop down list)

Tell us name, est date and customization txt (this needs to be a free form comment section)

Name (form field)

Address (form field)

Credit Card info (form field)

Pay Now (button)

Need it to process payment and send me an email with the info they gave.

Can jotform do this and tie it in with my paypal or something?
