If you want to modify the layout of the PDF submissions, you can do that using the "Customize PDF" feature under the "PDF" button in your submissions page:
Here you can add your log and footer.
Please note that this will create a FIXED PDF layout, meaning it does not adjust based on your submissions' content. It is similar to a paper print layout. Please also note that this will apply to all of your PDF submissions on that form, including the one attached on your email alert.
Once the customized PDF is created, you should be able to see it under the "Reports" drop down list where you can edit or delete it:

Jonsturm345Replied on January 10, 2017 at 12:45 AMWhen I go to Reports to create a new PDF Design report, it does not give me that option. Only options are several others like excel, csv, and several others.

Jonsturm345Replied on January 10, 2017 at 7:45 AMOK thank you. Also in adding an image for the header on the PDF customization, it only gives the option of a URL rather than uploading an image. Is there anyway to just upload a logo image?

Unfortunately, the only way to add an image to your customized PDF is by uploading the image to your server or any other third party image server and add the URL to your image in the PDF.
You can also upload the image here in Jotform using this link: https://www.jotform.com/help/image_upload.php?naked=true. You can follow the instructions below to upload an image in JotForm using the link that I have provided.
1. Go to: https://www.jotform.com/help/image_upload.php?naked=true
2. Click "Choose File" button and choose your image file (should be in JPG or PNG format and not more than 500KB)
3. Click "Send Image" to upload (you should be able to see the file successfully uploaded in the image section)
4. Select and Copy the direct file URL from the "Image URL" box
5. Add the Image URL to your PDF.

Hello Jon,
You can use any image server, Google Drive, DropBox, anything. And just copy the image URL and paste to the box.
