Form Builder looks different when compared with the Form Preview

Hi I have created this form:

but when publish the form only a former version of this form is shown in my brower. I also tried using in another browser to see if caching was the problem. 

I also tried to duplicate the form to https://form.jotformeu.com/70092386013349 but then I get the error: "Form not found This form has been deleted by the owner. Try contacting the owner of this form." 

What could be wrong here?

I checked your form and found no difference with how it looked like on the Form Builder compared with the Preview. If you could share us a screenshot of the differences, or at least point us which fields are different, we'll surely appreciate it.

I failed to see any difference with the original form:

They look exactly alike and from this, we can safely deduce you managed to clone the form properly.
