Hi,
We book a lot of training sessions through JotForm, once a form is submitted it gets sent directly to our inbox.
It has been pointed out that the email that gets sent to us including the feedback from the submitted report is wrong (I have tested this myself - comes back wrong every time).
The problem seems to be with the drop down selection boxes (we have a simple yes or no selection).
In a test submission I have put all these selections as yes. Although when it gets sent back to us, it either comes back as 'No' or blank.
I have created new drop down boxes and the problem still persists.
How do I resolve this?

I have inspected your form and found out that the "question tag names" in it do not match the ones you placed in your email notification.
I would suggest you use the "Form Fields" drop-down from the toolbar to fix the issue. (Please see screenshot below)
