I am looking to add a questionnaire to my squarespace website and want to know how your forms sync up with that platform.

Also, I want to create a questionnaire that asks questions about their skin history that depending on their answers will lead them to a PDF document that gives them possible causes and remedies. Is possible with your service?

This is achievable using the Condition Logic feature of the Form Builder.
