attaching a word document

I want folks to be able to download a word document (under budget), rename it, complete it, and then upload it to the form.  I know how to upload, but not how to attach the word document to the form.  Last year I did a link to our website and put the form there. Is that the only way to do it?  Many thanks, as always.
