Not all the files uploaded to our form are appearing in dropbox

Hi

We have a form set up named Young Artists Comp Application 2017 to collect images, doc files and pdfs which has just been filled in for the first time but only the pdf (ENSEMBLE AGAMEMNON.pdf) has appeared in our dropbox. I have checked that the Dropbox app has been integrated ok and it says it has been authenticated successfully. Is there something we are doing wrong or is there an issue. We would be very grateful for your help.

The form is supposed to forward uploads to your Dropbox along with the submission PDF file.
