I am the new Executive Director at the MCC Medical Clinic located in Silver Spring, MD.  We utilize JotForm of patient registration. We never signed a contract between Jotform and your Clinic. We usually would provide w-9 forms or filled out general vendor information if needed, but in general our users would rely in our services as we honor our terms of use and privacy policies.
