Your account was autosuspended by our phishing detector due to suspicious fields, which is why your form is disabled. Although upon checking your form, I don't see any violations of our terms. With that in mind, I have now removed the suspension on your account. Your form should be enabled now. Please refrain from collecting credit card info or login credentials to avoid getting suspended.
