    Form is not saving my changes

    I created a form and updated it and save it. The form looks great but when I preview it or use the link the form does not have my changes. The funny thing is that my imported photos appear but the fields are not up to date.

     

    See attached snapshot. Its missing Store Name

    We apologize for any inconvenience this may have caused you. Do you mean this field below? Because i was able to see it.

    Can you please change your forms URL from http://form.jotform to http://www.jotform and try again? Clearing your browser's cache would be good too.

    No the store and account names are missing. They show up in my template
    but when I use the link in my newsletter and click on it they are
    missing.
    I tried the new link as you suggested and I tried a different browser.

    Hi,

    I've check your form and I found that you created a conditions of Store Name field and Account # field. Can you please take a look on your conditions because some of the condition including said fields is not complete?
