    Online application is not coming through

    I am receiving notification that people have submitted a form, but the form itself is only coming through occasionally.

    This had been a problem a couple of months ago, but it had appeared to have been fixed. For the last couple of weeks the problem has returned, gotten worse, and persisted.

    Michael,

    I am not exactly sure if I get what you mean by this:

    I am receiving notification that people have submitted a form, but the form itself is only coming through occasionally.

    After checking your form though, I found out that you have two different notifications set up. The first one is incomplete and contains only a few fields; the other one has every single fields in it and has the JotForm logo.

    Are you saying that you always receive the first notification but the second one comes intermittently?

    If my assumption is correct, then the problem has something to do with your second notification's Sender Email settings.
