Why are our forms disappearing?

When we use the embed code--or even the iFrame code--sometimes the forms just don't show up. Sometimes they do. It's really frustrating, and we're wondering if anyone is having this problem. If so, how'd you fix the issue? 

Are you still experiencing this problem? There was a similar issue wherein the form is loading slow for some users. However, the issue is already resolved.

Upon checking your webpage, I was able to verify that it is working properly. There are no errors in the Console as well.
