I want to attach a "Worker Agreement Form" (PDF) and have users check that they have read and agree to the form and link to it?

    I have inspected your form and I saw that you have added Terms & Conditions widget to your form.

    This field is required by default and your users will not be able to submit the form, unless they check agree option. All you need to do is to add link to your PDF file.

    You can also embed your PDF file directly on your form with PDF Embedder widget. With this method you will need to add an additional Check box field to your form with option I agree to allow your users to agree with your terms and conditions.
