in setting up I was not directed to a page 2 that I saw............. however I have a 2nd page with someone elses info on it.....

I checked your form and I only see one page. It's also not set to redirect to another form. I also do not see any password reset instruction requested by you. What I did then is sent you a password reset instruction. Please check your inbox or spam folder for it.

I have not been getting any forms so I some investigating only to find the
attached additional pages when I personally inquired like a potential new
patient would. How do I as the owner of the jotform here put our info in
and not some random doctor who I have no knowledge of and isn't even in our
area. Again, see attached.
Below is the notification I was sent as the owner of headaheinquiry form at
the time the attached form was sent to chezzehed.

The reset link will be only sent to the email address that is associated with the account username.
In this case, the account username that host the form https://form.jotform.com/63548575753166 is "nokomischiro", and the associated email address is "firstname.lastname@example.org".
If you have access to that email address, simply check it's inbox, and go to the reset email that has been already sent, there you will find the reset link.

You seem to have multiple issues on your form. Can you please clarify those? We will need to separate the issues for us to assist you properly.
You've mentioned the second page on your form, but I don't see this. It's not even on your form revision history, although there are fields that are being removed from it. If you want to check, here's how: https://www.jotform.com/help/294-How-to-View-Form-Revision-History.
As for not getting "any forms", I think you mean the notification email. Is this correct? We're sending you the emails at email@example.com. Please check that. If you want to receive the form emails to a different email address, please follow this guide to change the recipient in the form:
As for the information from your submitters, well, you only have the Name, Email and Contact Number in the form. If you want to get more information from your submitters, you may consider adding more fields.
For the reset email, you can initiate it yourself. Please go to https://www.jotform.com/login. Click the reset password, enter your email address (firstname.lastname@example.org) and click Send Reset Instructions button.
