My file upload field in the form is not working

I have added a "form upload" option but it's not allowing me to actually add a file.  When I click on the "browse" nothing happens.  I've tried with both Explorer and Chrome. 

You seem to have removed the file upload in the form. Please add it again then let us know here so we can check it further.
