I have live forms on my website that capture my client sign ups and they have been down for two days now! I need them back up please ASAP!!!!!!!! Please advise why my account has been frozen and how i can be sure this wont happen again!

The account username Wolfmealprep is suspended because your form "W.O.L.F Meal Prep Recurring Member Sign-Up" is requesting credit card information in plain text fields, as shown below:
In light of this, it is a must to take any of the following options:
1. Integrate your form with any of our Payment integrations.
Dwolla integration, is in particular a great choice for USA based business, it offers to accept payments with no fee per transaction.
