Why is the form showing as disabled?

    I built two forms. One in the regular jotform and the other in the 4.0 and both even though they show in the template and as being integrated both are saying the form is disabled. I made sure that I clicked the published too.

    I have checked and your account is autosuspended by our phishing system, because of the forms you have created. Inside of all of your forms you are asking for different users login credentials and that is against our terms of use

    Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

    You need to delete all of your forms and to agree that you will not create any similar forms. Only then we can activate your account.

    I develop websites and such for my customers. I have not asked for any credit card information. I was trying to simplify things for both myself and my clients so that they have everything in one spot. The only person that would have access to it is myself and my client. When I am completed with it I send thier copy into a second drive that is a shared folder with just the owners email for accessibabilty. 

    Plus the drive that I have uploaded into my drive I have a two step verification for added security.

    Over 70% of the directories that I am setting up their company name on I am creating for them. The 30% that they may have created or had someone else created that needs updating. Most of the problems that I have seen in this industry is the lack of sharing that information with the owner causing grief for them at a later date. That is why I wanted a simplified form for them to use as well in the cases that they might have the info. 

    So with this information are we able to use the form? If not kindly reccomend what I could use and I will delete these for use on my other forms that I will need. Thanks

    As I mentioned in my previous reply it is against our terms of use to ask for user login credentials, such as password and login. 

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    We have a phishing system that is detecting this type of words inside of the forms and our system will immediately suspend any account, when those words are found. 

    We can activate your account if you delete your forms or all login and password fields from your forms. 

    If you need user login credentials, I can only suggest you to ask your users to provide them to you via email, by phone or by fax.

    Ok my dear,

    Would you kindly look at the forms now please? I altered all three. I think they would be in compliance now.

    If so please reactivate everything and I will check on them tommorrow on the 17th. Thanks. If not please let me know what if anything else needs to be changed.
