Incomplete payment notification in one of 4 forms set up for registration. The PDF form say accepted and when I check PayPal the payment is there. Should I be concerned? What gives?

Please first make sure that the IPN setting within your PayPal account is enabled:

    https://www.jotform.com/help/276-How-to-Enable-IPN-on-your-Paypal-Account

Then, please check your IPN history in PayPal, as described insection "Completing paid submissions" of the following guide:

    https://www.jotform.com/help/272-Paypal-Incomplete-Payments-FAQ

If there are no failed IPNs showing in your PayPal account's IPN History, then you do not need to be concerned about the notification you received. The notification is then simply informing you that someone submitted your form, but then changed their mind and didn't process the payment through PayPal.

If you need further clarifications or assistance, please let us know.
