    Hi!

I created a form using a theme for volunteer sign-up for a race. But I edited it to reflect my purpose which is Volunteer Sign-Up for Alumni (FLOGA) Committees.

I have 14 responses so far but as I went by the page which had the form, I saw that it had reverted to the old form which is totally unrelated to what I require.

My questions is, How is this possible?

The form has worked successfully for 14 submissions and suddenly it changes.

How can I get my form back? It will take too much ebnergy to start afresh creating a new form.

    Thanks!

Is there anyone else who might have had access to the account? I checked the activity logs and several changes were made by a user who logged in to your account. You can also check this on your end by following this guide: How to View Your Account Activity Logs.

I would suggest to use our Form Revision feature to revert your form to a previous version of it where you have made the correct edits. Please follow this guide on How to View Form Revision History
