I you "Have you had a previous skin diagnosis?

IF No - go to submit

IF Yes - fill the Previous Diagnosis & Previous Treatment

    THEN

For last question Have you had more Treatments

IF No - go to submit

IF Yes - ill the Other Treatments

    Etc

I have spent maybe 3 hours trying to get this to work and I have given up. Please advise if this is actually something your software can do

It appears that the current condition setup on your form simply hides the Page break when the user selects No. To make sure that your users are directed to the proper page, you will need to setup a "Skip to a page" condition. Please do not hide the page break. Use the skip to page condition instead. Here's a guide to help you set it up: How to Skip Pages Using Conditional Logic

    Example:

I cant get the conditional formatting to work Screenshot 20

Here's a demo form so you can see it in action: https://www.jotform.com/70148129691964 You can clone it to your account to see my setup by following this guide: How to Clone an Existing Form from a URL
