Questions regarding JotForm and paid plans

    Hi to whom it may concern,

     

    I'm interested in signing up with your paid user system.

    Few things I need to clarify before making payments:

     

    - I require responses from 100 UNIQUE respondents

    - Survey is done as an annual employee feedback so its important no names are involved

    - Survey not available to public only the company personnel

    - Preferably each person has a different login + password to track the respondents from different departments

    - Or each user have a unique one time password that's accessible to the survey.

     

    Will your bronze account be able to accommodate these requirements? 

    Or if you could give me suggestions on how to go about this survey please let me know ASAP.

    JotForm is a form builder that allows you to create forms for surveys, registrations, applications, etc...

    - I require responses from 100 UNIQUE respondents

    I am not sure what you meant by "unique respondents", if you want to limit users on your form, you can check the following links:

    - Survey is done as an annual employee feedback so its important no names are involved

    You will be creating your own form, you can remove any fields that would identify the user who submitted the form. 

     

    - Survey not available to public only the company personnel.  

    - Or each user have a unique one time password that's accessible to the survey.

    The forms can only be accessed if someone knows the form link. You can set an access code field using Conditional Logic. Here's a guide about conditional logic: https://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic

    We do not allow our forms to act as login page. We also do not allow fields that collect sensitive information like passwords, credit card details and the likes. Please check our terms of use page

    "Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information."
