First, thanks for helping me with my first question, the Add This code worked great. Now as far as that Add This code goes, I would like to know the following:
How do I get the "shared " information to say something different on Facebook? For instance, when you fill out my form it send an autoresponse email that contains the add this buttons. When I share on Facebook, the shared information has a title and a brief description. How do I change that information??
Do I use a META Tag like this: <meta name="title" content="Playmaker Source College Recruiting Questionnaires" /> AND <meta name="description" content="I just used the free College Recruiting Questionnaire form to get any college questionnaire for my sport." />in the jotform code somewhere or is this something that must be changed on my wordpress site? If this goes in jotform, where does it go? Thanks so much!!Page URL:

That should be done on your wordpress page. You do that with <meta> tags. Here's what Facebook says about this: https://developers.facebook.com/docs/share/, look for the "Configuring Meta Tags for Facebook Share" below.
