    Site says that I don't have any forms created. However, I obviously have
    an active form, as it is forwarded to an email address (and I'm billed for
    it monthly). Please advise.

    Hi Jay,

    I have checked the website (http://riverbend-counseling.com ) but I couldn't find any form in there. Could you please share the direct link of the form (embedded form link etc)?

    On the other hand, I have checked your account but I couldn't see any form in your account Caddoan .

    Maybe, you upgraded another account and created your form in it. We can find this account via Form ID of your form. Please share the URL that includes your form.

    I'll be awaiting for your reply.

    The form (http://www.jotform.us/form/42996641705161) belongs to the username shelby387. It is registered to the email address (skylar387@yahoo.com).

    The form may 'belong' to shelby387 but she was in my employ and developed the form for our use.  I am merely trying to get the forms sent to my email address rather than to the address to which they are currently being forwarded:  riverbendbilling@gmail.com.  They previously were sent to ealivaudais@hotmail.com and were sent to her.  When she left, the emails then were sent to riverbendbilling@gmail.com.  I only need to have the email notification come to me, I pay for it and own the website.  Please forward to jayljeter@gmail.com

    Hello Jay,

    I checked your payment history and found indeed that your email address jayljeter@gmail.com is the one paying for the subscription. I have now changed the email notification of the form to go to that address mentioned.

    However, may I recommend next time for you to instead log in and take control of your own account. By getting to your own account, it's likely better and advantageous for you security-wise which you can change all information found in it, including the yahoo.com email address which is currently set on your account settings. This is also better for you as you will have all the access to your forms, review your submissions, and change its settings, create integrations, etc. to your desire.
