I need information about use of Jot. For example, I want to customize a meeting request form.

The request form would be distributed internally at our company and routed to a director in change of facilitating the meeting that would eventually take place at a customer's office.  Since our questions are customized, do we need to purchase a subscription? Sorry, but I just do not know about Jot but was told to use it.
