Google drive folder creation is not working.

everything was working good until today. my forms are not creating the google folder, but are creating the sheet row. 

please help. 

I cloned your form and integrated it with my Drive. So far the integration seems to be working on my end. I am unable to replicate the issue.

Can you please try reintegrating the form with Google Drive? If the issue persists, please let us know. It would also help us further if you can share with us the link to the google drive where you are experiencing the issue.
