I created a form tonight. Two people who filled the form out from their phone got an error screen. I have a screenshot of one.
Also, I used the "file upload" feature and can't view the photos that people have attached.

I have test your last modified form and upload some files. I have checked your submissions and I didn't replicate any issue. Could you please check again?
