Fields on a newly created form were suddenly removed

I am having issue viewing a form i was working on today. Can you please assist.

I checked the form's revision history and I see that the fields were gone at the same time the conditions were added.

My suggestion would be is to go to the Revision History Tool of your form and revert to the time when there are still fields in the form.

After doing so, please try to re-add the conditions that will be removed after reverting.
