    I have two forms and it doesn't work in either.

    I have inspected your form http://www.jotform.ca/form/70125934761254 and you have set Visibility of your next and back buttons to hidden. Because of that they are not visible on the form and you can not complete the form.

    To resolve this issue set Visibility of your Next and back buttons to Visible.

    In case that you want to use Auto-Next option, please note that last field on the page needs to be radio button or dropdown feild.
