Why my conditions are not working?

    My conditions to hide fields when another field are not working.  They were several weeks ago.

     

    Please advise

    I have checked your form and found that you have some conflicts in your conditions, for example, the condition below shows that you're hiding and showing the same field with the exact same selection:

    The other conditions seem to be working fine, but I would suggest you to check them in order to find similar conflicts to the shown on the screenshot above.

    You could try deleting or disabling one of the duplicated conditions:
