need to create another form with the same exact fields that is NOT tied to
Dwolla. This 2nd form will only be used to capture data points and not $. I
attempted to do this by cloning the primary form, it's called '2017 CSA
Application - MAIL IN CHECK'. However, I can't figure out how to disconnect
DWOLLA from the form. Please advise.

You may delete the payment field from the form by clicking on X icon of the field or click on delete option from the quick menu by right-clicking on the field. Please see the screenshot below:

I need to capture all of the data fields currently outlined in the form.
This form is strictly for data collection, not payment collection.
Ultimately, when the person clicks 'Submit Order' I do not want them to be
directed to Dwolla since the person will not be submitting payment (they
will be mailing in a check instead). The person should only receive the
thank you page and email confirmation.
Therefore, how do I set the form up to only capture data and and direct to
Dwolla when clicking 'Submit Order'?

I understand that you want to collect the product information as well without redirecting the form to Dwolla. Is that correct? If so, you may consider using Purchase order field from the payment fields to replace Dwolla. Please click on X icon to delete the Dwolla field and make sure that you are clicking on Yes when it is prompted to keep the products.
When you add the Purchase order field to the form, the products will be added automatically. Since it is a Purchase order, there is no payment required and displays the Thank you page when the form is submitted.
