Creating a form with a data table

    Hi, I want to create a form that asks for two field of info at the top and then has a table below with 6 columns. So basically, an organization would add in their name and location at the top, and then enter in individual entry information for people on their team in the table below, so that all the data shows on the page at the same time and the data in the table is linked to the two bits of info (organization and country) entered at the top. Is it possible to do this in JotForm?

    Hello,

    As I have understood your question, adding a matrix table to your form will be helpful for your purpose.

    However, it is not possible to make "all the data shows on the page at the same time"

    Instead of this, you can create a form with textboxes for the name and location, send the form to each team. After receiving the answers from each team, you can show the submissions on your website as you can find it at the following thread: https://www.jotform.com/answers/268709-How-to-show-live-report-of-submissions-on-my-Website-page
