Thanks, Jot forms disabled and re-installed/activated in integrations - However... Jot Forms delivers to Drive but now leaves out the client name - can I change that ? Please assist and... thanks for the help so far..very impressed !Page URL:

I cloned your form and integrated with Google Drive. I sent a submission and I couldn't replicate the issue:
When I inspect your submission data, I see that the users didn't fill the company name field, hence it has no data in it. You have 4 submissions out of 25 submissions that have marked 'applying as company' filed. So please check again the drive if these 4 submissions also missing the company name field.
