How can I have the Unique ID widget to show up on the form view?

The Unique ID widget cannot be shown on the form. It is always hidden in the live form view. You may consider creating an autoresponder email and include the unique ID there:

 ...or place the unique ID on the thank you message page. 

Another workaround would be to use our Random Value Generator widget: https://widgets.jotform.com/widget/random_value_generator. You can hide/show the widget on the form view:
