Someone disabled my form. How is that possible, and how to fix it?

    I sent me form link to someone so she could try to copy my form, for her use. However, somehow she disabled my form. How can I fix it? I tried disabling and then enabling, and that did not work.

    Your account is currently suspended. Our phishing detector tool automatically suspended your account. I checked your form and I can see that you were suspended  because of the word "Facebook" in the title and in the first field. Collecting sensitive information is not allowed here in JotForm, please check our terms of use.
