Why won't my first question perform right?

We had a  form that stopped working and and cannot seem to get the form to perform like it is supposed to

I have checked the form in your webpage and seems you're using a custom frame to embed it, what I would suggest you is to try embedding your form using the iFrame code as given by the wizard.
