Why Are My Forms Saying Disabled?

To whom it may concern,

All three of my forms are saying that they are disabled even though they are published.

Hello Jay,

Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Syndication and Feedback Form" & "Review Commercial Pre-Production Specs $147" has fields like "Twitter Password" which makes it suspected of being used for phishing.

Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password field from your form and activated your account again.

Kindly refrain from creating form with email/password, login/password or credit card details.
