Can I create a form that customization and for sell?

Can  I create a document here that a customer can purchase and then upload images and information for customization and then download?

Thank you for your response. I have a healthcare "receipt" that doctors want to buy. It would need to be customized for their practice. I want this up for sale and then have it automatically customized by form fields or by HTML. I don't even know. I guess that's how it would work. Then it would automatically download with their info.
