My form is not optimized in iPhone

I have created my form but its not responsive on iPhone. Could you please help!!!! Really stuck!

I have checked your form and found what you have noticed on your end too, upon inspecting the web console I can see there are a lot of resources blocked as well as some script errors and mixed content errors too, this may be why some resources, such as the .css file that makes the form responsive, is not being properly loaded. 

What I would suggest you is to use the iFrame embed option instead, this way you will avoid this kind of conflicts, here's the guide: Getting-the-form-iFrame-code
