How can I upload a word document to make this into a jot form?

It says you need to upload it to a web page to get the URL but I cannot figure out how you do this, please advise?

Do you mean you want to allow your users to upload word document to your form? You can then add a File Upload field to your form and allow files with .doc and .docx extensions to be uploaded. Please see below.
