I've got to the page on Box where he gives me the option to Grant Access to JotForm, which I accept and I'm returned to Jotform website but it tells me the Authentication has failed and I should check my credentials, which are clearly correct because I'm logged on.
I have the account owner and admin. It is a business enterprise account.

I have cloned your form in order to test it on my side. I've added a Box integration to the form as you can see the screenshot below:
I have entered my credentials of Box account. I have seen that the integrations is ready and working properly. I have seen that there is no error in the integration as you can see below:
Moreover, I have fulfilled and submitted the form. I have seen the submission is saved into the Box account successfully.
Did you follow all of the steps that I have explained above establishing your integration?
All in all, I was not able to produce the issue you have.
Also, please clear your web browser caches and try once again. Because another credential could have stuck on the browser.
I have used Google Chrome and did not see any issue. Please try in another web browser.
On the other hand, it could be a temporary issue as it is working now.
Please test again and if the issue still persists, let us know.
