Why forms are not showing in my website?

Just to let you know that some forms are not displaying on my following Web pages:

Hello Patrick,

I checked URLs you provided and I found out that they are under another account: patvil4u. And that account is overlimit, because it is over the limit with form views. 

You can wait the next 1st of the month so your limits will reset, add the JotForm footer so you have the unlimited views (you can do that in the settings page - https://support.jotform.com/myaccount/settings), or you can upgrade your plan and have all your limits lifted.

Hi Patrick, 

As my colleague mentioned you exceed the form view limit in patvil4u; that's why, your forms was disappeared. You need to wait Feb 1st, 2017 to see your forms again or you need to upgrade your account (patvil4u).

However, as you mentioned that you cannot login to patvil4u. Could you please clarify the reason of why you are not able to login in that account?

#1- If you forgot the password of your JotForm account (patvil4u), you can reset your password, and then, login patvil4u. Please look at the following images:

Click Login>Forgot password?

Write account email address (associated with patvil4u). Click Send Reset Instruction.

#2- After that, you need to check your email inbox and click to reset link. If you are not able to login your email (associated with your JotForm account patvil4u), you cannot reset your JotForm password unfortunately.
