Why is my form disabled?

i create almost 3 forms now but none of them if found on my website ...it write page not found everytime ...what can i do pls

Upon checking your account I found that it's suspended by our anti-phishing system. As all your forms has fields like "Numero & Code Pin" which makes it suspected of being used for phishing.

It seems you are trying to create login form. Please check the screenshot below:

Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password field and activated your account again.

Kindly refrain from creating form with email/password, login/password or credit card details.
