Subscription name is not displayed in the notifications

In the JotForm notifications we receive for recurring donations, the amount is not listed, rather a dot (like a radio button) appears instead. (see below) How can I get the amount to display? Here's a link to the form: https://naamat.org/sustaining-partnership-program/ Thanks for the help!

This is happening due to the name of the subscription is left blank for all the subscriptions.

Please mention a name in the Name field of the subscription so that it will be displayed in the email notifications.
