    I have checked your account and forms. I realized that your form has contained "Credit Card Number", "CVV code" and "Expiration Date" fields.  You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use.  If you insist on collecting any of this information, your account will be suspended again.  

    I have deleted these fields and re-activated your account. Your forms are enabled now.

    Also, You can use our payment methods. It is available with PayPal, Stripe, and Authorize.Net integration.

    And I would suggest you check the guide below to learn more about enabling payment authorizations through those payment integrations.
