My form is longer showing up on my website

I'm having the same issue described above, I had a jotform on the following link :

http://clarkesofnorthbeach.co.au and it hasn't been displaying correctly for a little while.

You have exceeded your form views limit, that is why the form is currently not shown on your website. Free accounts are limited to just 1000 form views per month. Your current view is 1137. 1 visitor view on your website homepage is equal to 1 view to your form. 

I have just reset your form views. I would suggest moving the form on another page then just put a button on the footer so visitors can click on it to contact you through the form. This way, only people that intend to contact you will be counted on the form views.
