I have created a check box table (called Family History) which appears fine on my website. But when I download the submitted form the table is cut off and I can't see the last three columns on the right.
I've tried making the table and/or columns narrowing but the downloaded document still has the columns cut off.

Hi,
I checked your form but it doesn't have any submissions on it.
However, I cloned it and made a test submission where I was able to replicate the problem. Here's a screenshot of my PDF submission:
Here's a screenshot of the submission in my JotForm account:
Let me forward this to our back-end team to have it checked and fixed. For the meantime, you may also try using the Customize PDF feature, you can learn more about that here: https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report
Apologies for the inconvenience.

Hey possibilitiesclinic,
It's actually not a bug. You need to set the label alignment to top for the matrix fields through our PDF Customizer. It's a shame that it is not automatic but it's not that hard to use. If you just follow the steps that I will provide, you will be able to export PDF reports which will look like this:
First of all you need to go to the Customize PDF builder, through your form's submissions page;
From there you are free to come up with any design you can come up with. For label alignment, here is a screen cast;
I hope this information will satisfy your needs. Let us know if we can do anything else for you,
Cheers!
