A form that used to work is now not working

Hi. I'm suddenly having trouble with a form that was working perfectly well for that last 6 weeks or so.

Here is the link to the form: https://form.jotform.com/63434216543957. It currently has 56 submissions, but now when people submit they get an error message.

At first I got an error message, but now i'm getting "please wait" for forever.

I think this is related to the problem that we are experiencing a while ago. We've fixed it already. Please check again and let us know if the issue persists to your end. I have tested it, it works fine for me.
