    Check box not working

    Hello,

    The check boxes are not working properly.  When selected, the word YES appears when the form is processes. 

    Form name is Email Version Colorado Vital Record Order Form

    How do I fix?

    Are you referring to fillable PDF form, form embedded into an email or something else?

    Hi Mike,
    Thank you very much for getting back to me. Sorry for my delay in responding.
    I am referring to the Fillable PDF form.
    But, I think I solved the problem. I chose radio buttons rather than check boxes and it now works fine.

    I apologize for not understanding what you meant with testing your email and submitting tests and receiving my colleague's message as a result.

    Could you please show us a screenshot of the issue, or provide us with any information that would put is in perspective, so that we can assist you better? Thank you in advance and again apologies for not understanding what you mean.
