The forms don't work

I noticed that you have images which takes a while to load. Having huge image file sizes cause a slow loading of a page. I would recommend to reduce the size of the image in order to have a faster loading time. 

Also, there seems to have a number of errors on your page itself, not related to JotForm. To check, you can press F12 on your browser to open the web inspector/debugger. You may consult with your web developer over these errors
